What Is A Geriatric Assessment?
A geriatric assessment is a comprehensive evaluation designed to optimize an elderly person's ability to enjoy good health, improve their overall quality of life, reduce the need for hospitalization and/or institutionalization, and enable them to live independently for as long as possible.

Once we identify current problems or anticipated future problems in any of these areas we develop a comprehensive "Care Plan" which addresses all problems identified, suggests specific interventions or actions required, and makes specific recommendations regarding resources needed to provide the necessary support services.

A "Care Plan" is an outcome of the geriatric assessment and it is a plan of action for the future.


How Does the Consultation Work?
Fill out the form and we will contact you to schedule your telephone or in-home consultation.  The geriatric care manager will conduct an assessment interview to gather information and listen to any specific concerns you may have.  Following the  assessment the geriatric care manager will create a written plan that will be emailed to you detailing resources and recommendations for your aging parent. We will also place a follow up call to discuss the care plan and answer any questions you may have.  After the consultation you may request weekly or monthly follow up visits as needed.  Some families may not need the care manager once support services are set up until there is a crisis.

 
Fees:
Hiring a geriatric care manager is no different than  hiring an attorney to help with legal issues or an accountant to to help with tax problems.  Although geriatric care management fees are not covered by Medicare or Medicaid, some employers, insurance companies , health plans and long term care policies are beginning to cover these services.  Our services are $45.oo per 30 minutes.  Any e-mails or phone calls made in association to the consultation are not charged any additional fees.   While this may seem costly, bear in mind that our services will likely save you money by assessing precise needs and choosing the best services that will best serve your loved one now.
